• If your student has Special Needs Transportation (SNT) as part of their documented accommodation in their Individualized Education Program (IEP), you do not need to complete this form.
  • If your student is in Early Childhood (Preschool), you do not need to complete this form.
  • McKinney-Vento and Foster Care eligible students that are unable to submit a transportation request on the TV Portal, must contact their school liaison to certify their eligibility and access services beginning July 1, 2026.
